Description Responsible for providing direct patient care under the supervision of a registered nurse.
Responsibilities include following the plan of care, providing treatments, and working collaboratively with the members of the team to help meet positive patient care outcomes.
Responsibilities and Duties Patient Care Provides direct patient care as defined in the state Nurse Practice Act.
Implements plan of care initiated by the registered nurse.
Provides accurate and timely documentation consistent with the plan of care.
Assesses and provides patient and family/caregiver education and information pertinent to diagnosis and plan of care.
Assists the registered nurse or physician in performing specialized procedures and duties, which include preparing equipment and materials for treatments, observing aseptic technique as required, and assisting the patient in learning appropriate self-care techniques.
Participates in coordination of home health services, appropriately reporting the identified needs for other disciplines (HHA, OT, PT, MSW, ST, Dietician) to the registered nurse and/or Clinical Supervisor/Nursing Supervisor.
Uses equipment and supplies effectively and efficiently.
Participates in personal and professional growth and development.
Performs other duties within scope of practice, as assigned by the registered nurse.
Qualifications and Skills Basic
Qualifications:
Current and unrestricted LVN or LPN License in the State of California.
Current BLS certification.
Clinical experience in an inpatient or acute setting.
Problem solving skills; the ability to systematically analyze problems, draw relevant conclusions and devise appropriate courses of action.
Excellent verbal and written communication skills; ability to speak clearly and concisely, conveying complex or technical information in a manner that others can understand, as well as ability to understand and interpret complex information from others.
Able to lift, position or transfer patients.
Able to lift supplies and equipment.
Considerable reaching, stooping, bending, kneeling, or crouching.
Visual acuity and hearing to perform required nursing skills.
Intermediate computer skills - Proficiency with Microsoft Word, Adobe, and Internet Explorer.
Must be a licensed driver with an automobile that is insured in accordance with state/or organization requirements and is in good working order.
Must be able to pass a criminal background check and drug test.
Preferred
Qualifications:
1 year of home health experience.
Bilingual.
Availability to work weekends regularly.
Knowledge of or experience with the Briggs OASIS forms.
Experience in acute long-term care, acute rehabilitation, or skilled nursing facilities.
